The CSW offers two types of memberships, a free non-voting membership, and a paid voting membership. The fees for voting members are $99 per quarter. Also, for a limited time we are offering a Founding Membership.

If you would like to join please submit you clinic information below so you will be included in our directory, and check the box for voting or free membership at the bottom of the submission form. If you are interested in becoming a Founding Member please check that box at the bottom of the submission form. To become a Founding Member requires a one time donation of $500.
